Disability Claim
Disability means you are temporary disable, or totally and permanently disable confirmed by a registered Medical Practitioner.

- Disability Claim Form: Claimant – (to be completed by you)
- Disability Claim Form: Employer – (to be completed by your line manager or HR)
- Medical report: Doctor – (to be completed by your treating doctor. Please ask your doctor to include copies of all diagnostic test results (e.g., blood test results, x-ray reports etc) and any other relevant medical reports (e.g., physiotherapist, psychologist etc)

Retrenchment or Loss of Income Claim
Retrenchment or loss of income means becoming unemployed or the inability to earn your full income as a result of any action that is beyond your control and receiving no remuneration from any occupation. The loss of income does not include your voluntary termination of a contract, or any wilful steps, actions or decisions that lead to you not earning an income.
